Rationale
A responsive outpatient support system for persons who have experienced an acute psychiatric episode requiring hospitalization is essential to maintain clinical and functional stability and to prevent readmission to hospital.

  • Overall average is too general an indicator - this needs to be sub divided by need.
  • Actionability lower for PHC providers because may not be alerted to hospitalization. Higher for regional health authorities or community partnerships
  • Less under primary care control. Too simplistic; as an indicator of continuity it is misleading.
  • Few if any EHRs that would capture this in health facilities on FN reserves
  • Very relevant and easy to understand
